We are proud to be in-network with a large variety of PPO and HMO insurance providers including; Blue Cross Blue Shield, Aetna, Cigna, United, and Medicare . Once you fill out your new patient paperwork, we will run a verification of benefits to determine your coverage, and call you to discuss and schedule your initial evaluation.

Do You Accept Medicaid?

At Bespoke Psychiatry, we understand the inequality in healthcare access for individuals covered by Medicaid policies. In order to address this issue and ensure that everyone has access to mental health services, we have allocated specific days and providers exclusively for our Medicaid patients. To get started, simply click on the “become a new patient” link, complete your intake, and one of our team members will promptly verify your benefits and contact you to schedule an appointment.

Typically we are able to get patients scheduled with-in 7-10 days. You may also request to be put on the waitlist for sooner availability.

We are currently operating via telehealth while our office is being remodeled. Once complete, you may choose to solely be seen in-person or via telehealth or, do a combination of both to best accommodate your schedule.

Please click here and you will be redirected to your patient portal to request a refill or to message your provider with any concerns or questions. To make any medication changes or adjustments in dosage, we require an appointment to be scheduled to discuss.

This will be discussed during your initial evaluation. Typically, every 4-6 weeks, but you may need to do a one to two-week follow-up after your initial evaluation to evaluate any side effects. Please keep in mind that any medication changes may take 6-8 weeks to take full effect.

We understand situations happen that are beyond your control. Please call us if you will be running late. If you are running more than 15 minutes late, you may incur a late fee and you will need to reschedule your appointment.

We are on the corner of Augusta and Elston at 1203 W Augusta Blvd. Please enter through the front door on Elston. We are currently operating via telehealth while construction is being done in our office.

There is free street parking on both Augusta and Elston.

The number 56, 66, and 70 bus stops and the Chicago blue line stop are all within .4 miles of our location.
